Requirements for issuance of passport to a newborn child in India

• Application Form-“B” (Please click to download) duly filed in for New passport.
• Birth Certificate (duly attested by MEA).
• Copy of Pakistani Father and Mother’s CNIC/Passport.
• Copy of Residential Permit (RP)/Long Term Visa (LTV)/valid Indian visa.
• Copy of request filed with Indian authorities for an extension of visa (if applicable)
• Marriage Certificate of parents.
• ‘S’ Form (Please click to download) in case of newborn baby, whose father or mother is a foreign national.
• Affidavit from parents stating that the child has neither got any Pakistani passport previously nor he/she has been issued Indian Passport or any kind of documents of Indian citizenship.
