Requirements for obtaining a manual passport for below 12 years of age

• Application Form-“B” (Please click to download) for New passport in lieu of Expired or Exhausted passport.
• Original old/expired passport.
• Original Child Registration Certificate (CRC)/NICOP issued from NADRA along with a photocopy.
• Four current passport size (2X2, Front Face) latest photographs.
• Proof of residence in India (LTV/Any Indian Visa).
• Copy of request filed with Indian authorities for an extension of visa (if applicable).
• Affidavit from parents stating that he/she has neither got any Pakistani passport after the present one nor he/she has been issued Indian Passport or any kind of documents of Indian citizenship.
