Requirements for Passport (MRP)

Documents required for obtaining passport:

1. Identity Card (Valid) + Copy
2. Original passport + Copy
3. Omani Civil Card + Copy

For passports of applicants under 18 years of age (Minor):

1. Original CRC/Smart Card/NICOP + Copy
2. Original Birth Certificate (in case of first-time passport) + Copy
3. Valid Original Identity Card / Original Passport / Omani Civil Card of parents + Copy
4. Family Registration Certificate (FRC)
5. Custody / Guardianship Certificate obtained from family court in case of separation and death of parents. The Guardian must secure permission from the Court to issue passports in favour of children for travelling purpose.
6. Applicants under 18 must be accompanied by a parent
7. Passports for children below the age of 15 will be issued for five years only
8. In case, either of the parent is out of Oman, the passport will be issued subject to his/her permission only.

Passport under lost category:

1. Police Report of lost passport
2. Loss for the first time, double fee will have to be paid
3. Loss for the second time, four times the fee will have to be paid
4. Loss for the third time, the case will be sent to the Head Office of Passport Islamabad Pakistan and their clearance will be followed in the light of the instructions issued.

Note 1: NOC is required from concerned department in case of Government / Semi-Government or autonomous body employee.
