For Children 10 to 18 years
i. Children between 10 to 18 years of age must have Juvenile Card.
ii. Old Passport in case of renewal

iv. CNIC, Passport Copies and Residence card of both parents
v. Marriage Certificate of parents
vi. If the child is less than 18 years of age then both parents have to visit the embassy along with their child. In case if any one of the parents is unable to visit the embassy, then the affidavit of the parent, who is not accompanying the child is mandatory. "The affidavit must state that there is no separation or custody case filed in court. The affidavit shall be signed and thumb impression must be affixed". Children under 18 years of age will be issued passport 05 years validity.
(NOTE: If any one of the parents/both the parents are in Pakistan or any other country then the affidavit shall be duly attested by Foreign Ministry of Pakistan or the Embassy/Consulate in that Country).
Children below 10 years
i. Original computerized form-B/CRC and NICOP along with its photocopy (in case of applicant below 10 years of age).
ii. Residence Card
iii. CNIC, Passport Copies and Residence card of both parents
iv. Marriage Certificate of parents
v. If the child is less than 10 years of age then both parents have to visit the embassy along with their child. Passport Loss Case
i. CNIC
ii. Original FIR of the Passport Lost
iii. Passport Copy (If any).
iv. Passport Number (Compulsory).
v. Police report clearly mentioning the current passport number which is lost (Compulsory).
vi. In case of lost Passport, double fee will be required first time, second time lost it will be charged four times and in case of third time lost, approval of DG General Immigration and Passports Headquarter, Islamabad will be required.
IMPORTANT NOTE
i. Please show full accurate correct address (Present & Permanent) and contact number (Mobile) in the Form.
ii. In case of dual nationality, please show Passport of the Dual Nationality.
iii. Photocopies of any of the above document will not be accepted.
iv. Applicant presence is mandatory for taking picture & finger prints otherwise MRP cannot be processed.
v. NOC required from parent department for official persons.
vi. Asylum/Refugee/Political seekers cases are referred to IB for NS/Security clearance and after that applied MRP will be issued accordingly.
vii. All charges would double in case of issuance of passport against lost report.

Passport Delivery:

Emergency Travel Document
Emergency Travel Document (ETD) is issued to the Pakistani nationals for travelling back to Pakistan only in extreme emergency cases when their Pakistani Passport is not available OR they are not eligible for issuance of a new passport / renewal of passport.
Documents are required for issuance of Emergency Travel Document by the Embassy:
i. Copy of NICOP/Passport (both or one of them)
ii. Three photos
iii. Report from Police for missing passport
iv. ETC Application form available on Embassy website
v. Air Ticket
vi. Proof of urgency
vii. Minor should be accompanied by either of the parent’s along with parents original CNIC and Passports.
viii. Duly signed consent form of parents in case of minors
