NADRA only accepts online applications for preparation of NICOP, POC, Juvenile Card (JV), Child Registration Certificate (CRC), Family Registration Certificate (FRC) and Cancellation Certificate. For online submission of application, please visit NADRA website at the following link:

III)   Instructions for issuing miscellaneous certificates

Nationality Certificate

i.      Original Pakistani Passport

ii.     CNIC copy

iii.    Zairyu card copy

iv.    Duly filled Application Form (available on Embassy website)

v.     Fee Yen.1000/-

Driving License Registration Certificate

i.      Original driving license

ii.     Copy of valid Passport

iii.    CNIC copy

iv.    Zairyu card copy

v.     Fee Yen.1000/-

Marriage Registration Certificate

i.     Original Marriage Certificate, issued, within period of last three  months, by concerned City Government of Japan.

ii.     Copies of Valid Passports of husband and wife, (copy of valid CNIC, in case of Pakistani husband or wife).

iii.     Copy of Zairyu Card (in case of Pakistani husband or wife).

iv.     02 passport size photographs (both of husband and wife).

v.      Fee Yen.3000/-

Spelling Certificates of children

i.     Kosikotun Letter from City Government

ii.    Copy of Valid Pakistani Passport & CNIC

Child Birth Registration Certificate

i.     Duly filled S-1 form (embassy website)

ii.    Child Birth Certificate

iii.   Parent’s residential certificates (juminhyo)

iv.   Copy of Koskitohan (if mother’s nationality is Japanese)

v.    Both parent’s passport copies

vi.   Their CNIC copies

vii.  Zairyu Card copies

viii. One picture (passport size)

ix.   Fee Yen.1000/-

For issuance of any kind of Certificate, Application Form + Fee are required.

IV )    Instructions for Attestation

  • Any Document including marriage certificate issued from Pakistan can only be countersigned by the Embassy, if it is attested by the Foreign Office in Pakistan either from Islamabad or from Camp Offices in four provinces.
  • Documents issued from Japan can be attested by the mission if attested by the Japanese Foreign Office.
  • The documents to be attested should not be older than three (3) months.
  • For Attestation of the Power of Attorney, the Executant (Principal) has appear at Embassy. The Power of Attorney must bear signatures of 2 witnesses with copies of their passports/CNIC including 2 pictures of Executant, copies of his/her valid passport, CNIC and Zairyu Card.

  • Education Certificates/ Degrees must be attested by HEC and MoFA before submission.

    -  Proof of Registration (POR) for Afghan Nationals
    -  All types of Affidavits
    -  Disability Certificate
    -  Food Business License Certificate
    -  Trade-Mark Registration
    -  Service/Experience Certificate & NOCs on Posting/Study Leave Abroad
    -  British Educational Certificates pertains to Maddaras
