Marriage Registration Certificate

i.          Original Marriage Certificate, issued, within period of                last three months, by concerned City Government of                  Japan.

ii.         Copies of Valid Passports of husband and wife, (copy                 of valid CNIC, in case of Pakistani husband or wife).

iii.        Copy of Zairyu Card (in case of Pakistani husband or                  wife).

iv.         02 passport size photographs (both of husband and                  wife).

v.          Duly filled General Application Form (available on                       Embassy website)

Spelling Certificates of children

i.            Kosikotohun Letter from City Government

ii.           Copy of Valid Pakistani Passport & CNIC

Child Birth Registration Certificate

i.            Duly filled General Application Form (available on                      Embassy website)

ii.           Duly filled S form and S-1 form (embassy website)

iii.          Child Birth Certificate

iv.          Parent’s residential certificates (juminhyo)

v.           Copy of Koskitohan (if mother’s nationality is                              Japanese)

Power of Attorney 

For Attestation of the Power of Attorney, the Executant (Principal) has to appear at Embassy. The Power of Attorney must bear signatures of 2 witnesses with copies of their passports/CNIC including 2 pictures of Executant, copies of his/her valid passport, CNIC and Zairyu Card.
