Guidelines / Requirements for Attestation of Documents

 

Attestation of Power of Attorney / Mukhtar Nama

1. Requisite text should be printed on plain/legal paper alongwith its photocopies.
2. In case of Malaysian National, it should be notarized from Notary Public and attested by Ministry of Foreign Affairs, Malaysia.
3. Executant(s) and witnesses are required to come in person with original NICOP/CNIC and Passport to sign on POA in presence of authorized official of High Commission.
4. Copies of CNIC and Passport of Executant(s) & Witnesses
5. Three passport size photographs of executant
6. A copy of ownership papers of property (In case of Power of Attorney for property).

Attestation of Power of Attorney executed from Pakistan

1. Power of attorney issued from Pakistan should be attested by Ministry of Foreign Affairs, Islamabad or its Camp Offices in Peshawar / Lahore / Quetta / Karachi.
2. A copy of the above power of attorney (For the record of High Commission)
3. Copies of CNIC/NICOP, Passport and Visa.

Divorce Notice/Talaq Nama

1. Requisite text should be printed on a plain /legal paper alongwith its photocopy.
2. Executant is required to come in person with original NICOP/CNIC and Passport to sign on POA in presence of authorized official of High Commission.
3. Copies of CNIC, Passport & Nikah Nama/Marriage Certificate.
4. Two passport size photographs
5. Presence of two witnesses and their CNICs copies.

Affidavit/Authority Letter/Undertaking/Declaration etc

1. Requisite text should be printed on a plain /legal paper alongwith its photocopy.
2. Executant is required to come in person with original NICOP/CNIC and Passport to sign on Affidavit/Authority Letter/Undertaking/Declaration in presence of authorized official of High Commission.
3. Copies of CNIC & Passport Executant & witnesses

Attestation of Nikah Nama / Marriage Certificate/Birth Certificate

1. Original Nikah Nama/Marriage Certificate/Birth Certificate should be attested by Ministry of Foreign Affairs, Islamabad or its camp offices in Quetta / Karachi / Peshawar / Lahore (Pakistan) alongwith its photocopy.
2. Copies of CNIC/NICOP & Passport.

Family/Relationship Letter

1. Copies of Passports & visas of family members
2. In case of married female, copy of Nikah Nama / Marriage Certificate is required.
3. Copy of birth certificate (In case of dependent children)

Marriage NOC

1. An Affidavit by parents or 1^stwife (in case of married) regarding marital status & permission for marriage/2^ndmarriage or Unmarried Certificate is required which must be attested by Ministry of Foreign Affairs, Islamabad, or from its respective Camp Offices in Karachi, Lahore, Peshawar and Quetta.
2. Copies of above-mentioned Affidavit/Unmarried Certificate & Passport
3. IC copy of fiancé.

Educational Documents

1. Original educational documents need to be attested first by HEC or IBCC, and Ministry of Foreign Affairs, Islamabad, or from its respective Camp Offices in Karachi, Lahore, Peshawar and Quetta.
2. Copies of educational documents for the record of High Commission.
3. Copies of Passport and visa.

Pay and Pension related Papers/Life Certificate

1. Duly filled-in Pension form/Life Certificate and its copy
2. The Executant is required to come in person with original NICOP/CNIC and Passport to sign on Pension Form/Life Certificate in presence of authorized official of High Commission.
3. Copy of Passport & CNIC

Miscellaneous Certificates/Letters

1. Copy of Passport & CNIC
2. Copy of relevant document

All other Pakistani documents

All other Pakistani documents should be attested by Ministry of Foreign Affairs, Islamabad or its camp offices in Karachi / Quetta / Lahore / Peshawar before attestation from this High Commission

Commercial & Business Documents

1. Business/Commercial Documents or any other document on trade etc. of Malaysian Companies/firms are required to be notarized by Notary Public Office and countersigned by the local Foreign Office.
2. A complete set of photocopies of all the documents to be attested.
3. Authorization letter by a firm/company owner for submission and collection of documents alongwith his/her IC copy.
4. Copy of IC of authorized representative

All other Malaysian documents

All other Malaysian documents should be attested by the Ministry of Foreign Affairs, Malaysia.

Note:    Doubtful documents will be referred to the Ministry of Foreign Affairs, Islamabad or its camp offices in Lahore / Karachi / Quetta / Peshawar for verification (Applicant would be informed accordingly).

Machine Readable Passports (MRP)

Process Flow for Machine Readable Passport (MRP) Application/Processing

Step 1: Present fee deposit receipt, valid CNIC/NICOP and previous Passport (Declaration of Lost Passport Form in case of loss of Passport) alongwith photocopies are required for submission of application for passport. You will be issued Queue Number if documents are complete.

Step 2: You will be called by Queue Number to capture digital photo (without eyeglasses) and fingerprint at Counter No.1.

Step 3: You will be called by name for data entry (Please Read Data Form carefully and sign) at Counter No.2

Important Notes:

• No need to make an appointment. Applicants will be entertained on the basis of “First Come First Serve Basis”.
• MRP can't be issued or renewed in mail or with any representative.
• No need of any form and photograph.
• No amendment/modification/extension is permissible in Machine Readable Passport. The Applicant should apply for a new Passport for modification.

Applying for New or Renewal of Machine Readable Passport

Machine Readable Passport (MRP) system uses the data from National Database and Registration Authority’s (NADRA) system and MRP application can only be processed if applicant has one of the following valid original documents issued by NADRA:

1. Computerized National Identity Card (CNIC)
2. National Identity Card for Overseas Pakistanis (NICOP)
3. Smart National ID Card
4. Computerized Child Registration Certificate (CRC/Form B) for under 18 years applicants only. If you do not have a valid CNIC, NICOP, Smart National ID Card or CRC please apply for one of these NADRA documents and proceed for MRP application after you have received it.

Requirements for Adults' (18 years and above) application processing

1. Personal appearance of the applicant
2. Previous Pakistani Passport alongwith its photocopy
3. Production of one of the following original valid NADRA Card along with its photocopy:
   1. Computerized National Identity Card (CNIC)
   2. National Identity Card for Overseas Pakistanis (NICOP)
   3. Smart National ID Card

Notes:

1. Married female applicants are required to produce original CNIC / NICOP/Smart National ID Card containing their husband’s name. They would otherwise be required to get their CNIC / NICOP / Smart National ID card modified in accordance with their marital status before applying for MRP.
2. Pakistani Government employees are required to produce original NOC issued within last ninety (90) days by their respective Ministries / Divisions / Departments. 

Requirements for Children (below 18 years) application processing

1. Personal appearance of the applicant
2. Applicants under 18 years of age must be accompanied by the parents/guardian.
3. Production of one of the following original valid documents along with its photocopy:
   1. National Identity Card for Overseas Pakistanis (NICOP)
   2. Computerized Child Registration Certificate (CRC/Form B) issued by NADRA
4. Both Parents/guardian Original Pakistani Passports and NICOP/Smart ID Cards alongwith with photocopies.
5. Detailed birth certificate containing both parents' names on it and its copy.

Note:

In case parents are separated or divorced, the custodian father or mother is required to produce court orders confirming his/her legal right of custody of the child.

Online Passport Renewal

The Government of Pakistan has launched an Online Passport Renewal System through E-Service. All applicants are informed that the system is just for “Renewal of Passport”. All other applications will be handled by the MRP Section of the High Commission as per existing practice. Visit the following link for further information
