S-1 Birth Registration Form

The Consulate General of Pakistan, Milan provides the S-1 Birth Registration Form for children born abroad to Pakistani parent(s). The S-1 Form serves as a consular record of the child's birth and may be required for NADRA and other official purposes in Pakistan.

Required Documents

Applicants are required to provide the following documents:

• Child's NICOP

• Foreign Birth Certificate of the child, clearly mentioning:

  • Name of the child

  • Name of the father

  • Name of the mother

• Valid Passport of the Father

• Valid Passport of the Mother

• Valid Residence Permit (Permesso di Soggiorno) of the Father

• Valid Residence Permit (Permesso di Soggiorno) of the Mother

Important Information

• The child's name and the names of both parents must be clearly mentioned on the foreign Birth Certificate.

• Applicants should bring the original documents along with photocopies for verification.

• The particulars mentioned in the Birth Certificate, NICOP and parents' identity documents should be consistent.

• In case of any discrepancy in the documents or particulars, additional documentation or clarification may be required before issuance of the S-1 Form.
