3. Renewal of NICOP

Required Documents

• Valid Pakistani or Foreign Passport.
  
  
• Valid Residence Permit (if applicable).
  
  

Note: Renewal does not permit any change in personal information.

4. Reprint of NICOP (Lost/Damaged)

Required Documents

• Applicant must be present.
  
  
• Existing NICOP number.
  
  
• Valid Pakistani or Foreign Passport, or a copy thereof.
  
  
• Valid Residence Permit, or a copy thereof.
  
  

No attestation is required.

5. Modification / Correction of NICOP

Depending on the requested modification, the following documents are required.

Name Change

• Existing NICOP
  
  
• Birth certificate with correct NAME issued by the union council
  
  
• Name Change Deed/Undertaking duly issued and attested by the Consulate.
  
  
• Valid Pakistani or Foreign Passport.
  
  
• Valid Residence Permit.
  
  

Date of Birth Correction

• Existing NICOP
  
  
• Birth Certificate issued by union council if the requested difference of DOB is less than 5 years, In case of DOB change request of more than 5 years a court decree is requried.
  
  
• Passport
  
  
• Educational Certificates (if available)
  
  
• Any supporting government-issued documents establishing the correct date of birth.
  
  

Parent Information Correction

• Existing NICOP

Address Change
