Attestations & Consular Services

Authority Letter (General Power of Attorney)

Requirements:

Visit the Embassy personally with the following original documents:

Valid Pakistani Passport
Valid CNIC
Valid Residence Card/Visa

Fee: 9 Euro (to be paid via local bank card; cash not accepted)

Child Registration

Requirements:

Parents must visit the Embassy with originals and photocopies of:

Valid Pakistani Passports
Valid CNICs (wife’s CNIC must include husband’s name)
Valid Residence Cards/Visas
Child’s Birth Certificate issued by IRN/Civil Registration Office in Portugal
Two (2) photographs of the child

Additional Requirements:

If a parent is absent, a NOC authorizing the other parent to register the child is required. Must be attested by MoFA Pakistan or concerned Pakistani Mission abroad.
If a foreign parent does not hold PoC, a NOC is required for child registration. Must be apostilled or attested by the Diplomatic/Consular Mission of the parent's country.

Life Certificate

Requirements:

Visit the Embassy personally with originals and photocopies of:

Life Certificate (duly filled)
Valid Pakistani Passport
Valid CNIC/NICOP
Valid Residence Card/Visa
Pension Book (if applicable)

Fee: 9 Euro (to be paid via local bank card; cash not accepted)

Power of Attorney

Requirements:

Executant(s) along with two witnesses must visit the Embassy with:

Draft of the Power of Attorney
Two (2) photographs of the executant
Valid Pakistani Passport, CNIC, and Portuguese Residence Card/Visa of the executant
Valid Portuguese contact number of the executant
Two (2) witnesses with valid Pakistani Passport/CNIC and one photograph each
Copy of valid CNIC of the Attorney with contact number in Pakistan
Pakistani-origin Portuguese citizens without PoC must get the Power of Attorney apostilled by Portuguese authorities

Fee: 31 Euro (to be paid via local bank card; cash not accepted)

Citizenship Renunciation

Requirements:

Submit Application Form X (duly filled) along with six sets of the following:

Photographs (6x) with white background (2” x 1½)
Photocopies of Portuguese Passport & Citizen Card
Original Pakistani CNIC(s)/NICOP(s)

If lost, submit Police Report
If unavailable, submit Affidavit with Pakistan address

CNIC/NICOP Cancellation Certificate from NADRA
Original Pakistani Passport(s) (if lost, submit Police Report)
Two references with addresses and contact numbers in Pakistan
Photocopies of Passport, CNIC/NICOP/Form-B, or Foreign Passport of minor children residing abroad

General Guidelines for Document Attestation

Documents apostilled by MoFA Pakistan do not require further attestation from the Embassy of Pakistan, Lisbon.
Embassy attests only documents originating in Pakistan that are attested/QR coded by the Ministry of Foreign Affairs, Pakistan.
Embassy attests third-country documents if attested by the Pakistani Embassy/Consulate General in that country or by its Embassy/Consular Mission in Lisbon.
Embassy attests Portuguese-origin documents provided they are apostilled.

Passport Requirements

Passport Renewal for Child below 18 Years

To be accompanied by Parents, along with original Pakistani Passports, CNICs, Portuguese Residence Cards/Visas and photocopies.
Child’s Old Passport (Original + Copy)
Child’s NICOP and Form-B (Original + Copy)
Child’s Portuguese Resident Card/Visa
Original and Valid CNIC/NICOP is mandatory for the child of 10 years old or above.

Note:

NOC from the parent who is not present in Portugal must be attested by the Ministry of Foreign Affairs, Pakistan or concerned Pakistani Embassy/Consulate General abroad.
A foreign parent not holding PoC must provide an NOC for issuance of Pakistani passport to the child, apostilled or attested by the Diplomatic/Consular Mission of the parent's country.

Passport for Newly Born Child

To be accompanied by Parents, along with original Pakistani Passports, CNICs, Portuguese Residence Cards/Visas with photocopies.
Child’s NICOP (original + photocopy)
Child’s Birth Certificate issued by IRN, Portugal.
Child’s Birth Registration Certificate, issued by the Pakistani Embassy in Lisbon.

Note:

NOC from the parent who is not present in Portugal must be attested by the Ministry of Foreign Affairs, Pakistan or concerned Pakistani Embassy/Consulate General abroad.
A foreign parent must provide an NOC for issuance of Pakistani passport to the child, apostilled or attested by the Diplomatic/Consular Mission of the parent's country.

Note: In case of three times loss of passport, the passport will be issued after necessary approval from competent authority in Pakistan. Police Report along with written travel history and justification must be submitted.
