Power of Attorney Attestation

• Attestation of documents such as Power of Attorney and Authority Letter should be signed by the executant in front of the Embassy officials.
• All documents originating from Pakistan must be signed from the Ministry of Foreign Affairs, Islamabad or Foreign Office Camp Offices (Peshawar, Lahore, Quetta, and Karachi).

Requirements for Attestation:

• Original documents along with their photocopies.
• Original Pakistani Passport and Computerized National Identity Card (CNIC) or National Identity Card for Overseas Pakistanis (NICOP).
• All documents issued from Pakistan must be attested by the Ministry of Foreign Affairs or any Foreign Office camp offices in Pakistan.
• Personal appearance before the Consular Officer is necessary to sign Power of Attorney or other documents involving attestation of signatures. Please bring along your original ID and Passport. Plus two witnesses are required to sign Power of Attorney. Their ID and Passport copies are also required.
• All Portuguese documents must be attested by the Portuguese Foreign Ministry.

Required Documents

1. Application “Form X” (3 Copies) duly filled by the applicant.
2. Six photographs with white background of size “2 x 1 1/2”.
3. Three copies of Portuguese Passport
4. CNIC/NICOP in original with 3 photocopies. In case of lost CNIC/NICOP, lost report from local police in Portugal is required. If a person does not have a CNIC/NICOP he has to submit the following:
I. An affidavit stating that he does not have a CNIC or NICOP and mentioning applicant’s complete address in Pakistan.
II. Two references along with their addresses and contact numbers in Pakistan.
5. Original valid or expired Pakistani passports with 3 photocopies. In case of lost passport, lost report from local police in Portugal is required.

Note: Fee Euro 5 Renunciation Certificate + Euro 94 CNIC/NICOP Cancellation +Euro 92 POC processing fee.
