Attestation of Documents: Requirements

Attestation of Power of Attorney :

1. Original Power of Attorney, which is to be signed in the presence of an authorized official of the High Commission
2. Copy of NICOP or CNIC
3. Copy of Passport
4. Copy of Ghanaian Residence Permit / visa
5. Copy of NICOP / CNIC / passport of the witnesses
6. Please bring photocopies of above documents
7. Two photographs of deponent
8. Fee US $40/- for each Power of Attorney
9. Consular Service application form. Duly filled in and signed.

Note: - The deponent and two witnesses (Pakistani citizens) mustcome to the Mission in person with original identity cards, passports, Ghanaian Residence Permits / visas and sign in the presence of the consular officer vide Ministry letter No. Con (II)-Misc/2010 dated 22nd July, 2010.

Attestation of Invitations:

1. Original Invitation letter
2. Copy of Passport of invited person
3. Copy of Incorporation certificate of the company
4. Please bring photocopies of above documents
5. Fee US $20/- for each paper
6. Consular Service application form. Duly filled in and signed.
7. OriginalCNIC/NICOP,passport,GhanaianResidence Permits/visas

Attestation of other documents:

1. Copy of NICOP / CNIC of applicant
2. Copy of the applicant’s Passport
3. Please bring photocopies of above documents
4. Fee US $20/- each document
5. Consular Service application form. Duly filled in and signed
6. Original CNIC/NICOP, passport, Ghanaian Residence Permit / visa

Attestation of Ghanaian Educational certificate & Degree:

1. Copy of Passport of applicant
2. All the educational certificates and degrees must be attested from Ghanaian Ministry of Foreign Affairs, Accra
3. Please bring photocopies of above documents
4. Fee US$ 20/- each certificate and Degree
5. Consular Service application form. Duly filled in and signed
6. Original CNIC/NICOP, passport, Ghanaian Residence Permit / visa

Attestation of Pakistani Educational Certificate & Degree:

1. Copy of NICOP/CNIC of applicant
2. Copy of Passport
3. Original certificate up to FSc / FA must be attested from Inter Board Committee, Islamabad and Ministry of Foreign Affairs, Islamabad
4. Original Degree BA, BSc, B. Com, MBBS, Engineering, M. Phil, Ph D must be attested from Higher Education Commission, Islamabad and Ministry of Foreign Affairs, Islamabad
5. Please bring photocopies of above documents
6. Fee US$ 20/- each certificate and Degree
7. Consular Service application form. Duly filled in and signed
8. Original CNIC/NICOP, passport, Ghanaian Residence Permit / visa

Birth Registration and Endorsement:

• Duly filled in application Form called Form-Sfor Birth Registration.
• Letter of Consent/No. Objection Certificate (will be provided by the consular section and both parents will have to visit the consular Section and sign the document in the presence of a consular official at the High Commission).
• Parents must show the following original documents to consular office and provide a copy of each:

1. Birth Certificates bearing the name of the child and the names of the parents with photocopy
2. Passport and CNIC of parents with photocopy
3. Ghana Residence permit of parents with photocopy

• 02 photos of the child for registration.
• Fee US$ 20/- for child registration certificate
• Consular Service application form. Duly filled in and signed.
