REGISTRATION OF CHILD WITH THE EMBASSY

1) Applicant must be present in person.

 2) Duly filled-in Form S.

3) Five photograph of child with light blue background.

4) Birth Certificate duly attested by Syrian Ministry of Foreign Affairs along with English Translation (Original).

5) Copies of Pakistani valid Passports and CNIC/NICOP of parents having marital status written (original).

6) Copy of parents’ residence card in country of stay.

7) Marriage certificate of parents duly attested by the relevant Pakistani authorities or Syrian Ministry of Foreign Affairs.

Requirements:-

• Applicant himself or his blood relative can appear for attestation of documents. In case the applicant or his blood relatives are not available, authority letter is required for attestation of documents. Authority letter must be verified from Ministry of Foreign Affairs, Islamabad or its camp officer or concerned Pak Mission Abroad.
• Documents originated in Pakistan must be attested from Ministry of Foreign Affairs, Islamabad or its camp offices.
• Documents originated in Syria must be attested by Ministry of Foreign Affairs, Syrian Arab Republic
• Documents originated Abroad must be verified from concerned Pak Mission
