Life Certificate Attestation

Note: Life Certificate attestation is free of charge.

1. In-Person Attestation at the Embassy:

– Visit the Embassy in person with your original and valid CNIC/NICOP.

– Obtain the Life Certificate form from the counter, complete it, and provide your signature and thumb impression.

– The designated staff member will attest the form on your behalf.

2. Attestation by Mail:

• Download the Life Certificate form from the Embassy’s official website: embassyofpakistanusa.org.
• Complete the form, sign it, and add your thumb impression.
• Have the form notarized by a Notary Public.
• Mail the notarized form along with copy of your valid CNIC/NICOP to the Embassy along with a self-addressed USPS return envelope.

Attestation of Affidavits, Wakalatnama, Undertakings, etc

i. Visit the Embassy in person with your original Pakistani CNIC or NICOP.

ii. Sign the document and place your thumb impression in the presence of the Embassy Officer.

iii. The attestation fee is US $32 per document (if paid through the card machine) or US $30 per document (if paid by cheque made payable to the Embassy).
